We'll then My Online Perk Club is the perfect solution to tackle your Close To My Heart wish list and earn FREE products. There are several clubs to choose from to meet your budget and needs. (see below)

Why Join My Online Perk Club?

• Get a FREE CTMH Idea Book just for joining My Online Perk Club!
• Receive a special goodie from me when your club membership ends as a special thank you!
• Refer a friend to My Online Perk Club and receive a $10 product credit when they complete their club term. (no limit, refer as many friends as you'd like)
• Receive $25 (or more) in FREE Close To My Heart products

How do I join and what else do I need to know?

• Choose the Club from the list below that best fits your budget and needs.
• Fill out the form below and I will mail you the current CTMH Idea Book!
• Please note the club minimum that you agree to is BEFORE shipping and tax.
• Be sure and have your orders submitted online at www.CTMH.com/Mandy by the 5th of each month!
• You will earn your $25 Perk Club Rewards during the final month of your club term. During this final month you may chose to gather more orders from friends and family to increase your order total to earn more FREE products. Check out the chart below for the reward levels.
• If for whatever reason you have a month that you can not place an order, please contact me prior to the 10th to let me know and I will extend a 1 month pass to you.
• If you find you can not continue your membership, you may cancel your club membership at any time. However, you will forfeit your earned Hostess Rewards! (you will still receive your referral product credit when referring a friend who completes their club term)
• During any given month of your club term you may submit an order of $150 or more and you will earn the additional Hostess Rewards on this order. Make sure you submit this order directly to me and NOT online in order to claim the Hostess Rewards!
• Orders are shipped right to your door via FedEx. After your order is placed you will receive a FedEx tracking number for your convenience.

• Club A: Spend $15 a month for 10 months
• Club B: Spend $20 a month for 8 months
• Club C: Spend $25 a month for 6 months
• Club D: Spend $30 a month for 5 months
• Club E: Spend $50 a month for 3 months

Whichever club you decide is best for you will receive your FREE hostess rewards during your final club month. Example: Join Club D and receive your $25 in FREE products during your 5th month.
